How can I get my loved one to agree to attend the center?
Many of our Family Members/Caregivers refer to their loved one as coming to a “social club” or “work”. Once you bring your loved one here we will take care of getting them comfortable and introduce them to clients of similar interests to make them feel at home. Once your loved one becomes familiar, they will look forward to attending our center, engaging in activity and seeing their new friends.
What are adult day services centers?
Adult Day Services Centers are designed to prevent premature institutionalization of older adults who are physically or mentally impaired by offering a community-based alternative, thereby allowing persons to remain in their own home. Centers also provide health maintenance services, social interaction, and respite for caregivers.
What type of transportation is offered?
Our hours were developed to assist working caregivers; therefore most of our clients are transported by family members. However, for those requiring transportation, SEPTA CCT Connect and Community Transit are available.
Who is eligible?
To be eligible to attend an Adult Day Services Center, an individual must be a resident of Pennsylvania who meets the following criteria:
-Sixty years of age or older
-Eighteen years of age or older and has post-stroke dementia, Parkinsonism, or a dementia-related condition such as Alzheimer’s disease or Traumatic Brain Injury.
-The individual requires assistance to meet personal needs and perform basic daily activities.
What type of safety protocol is there for Family Matters ADC?
Our center has implemented many safety precautions to prevent the potential of an unescorted egress by any client that ensures the safety and supervision of each client in the centers charge. (i.e. supervision of arrival and departure, checking for appropriate position of contact alarms on each door, calculating current time census through the day, practice of “head counts” throughout the day)
What are my payment options?
County Office of Services for the Aging (COSA), Veteran’s Administration, Liberty Resources (under 60), Long Term Care Insurance and Private Pay.
How do I sign my loved one up?
Come check us out! Give us a call for information or stop in unannounced for a tour as we have an open door policy and staff onsite at all times to provide you with information. Prior to enrollment, each client must thave their PCP complete a History and Physical form, inclusive of an updated PPD (TB test). Click here to download a copy of the form. The next step is meeting with the Program Director or Program Specialist to complete an Enrollment Application, as well as a preliminary needs assessment and personal interview. We will then develop an Individualized Plan of Care, based on your loved ones needs, to promote their independence and maintain current level of functioning.
